Quarterly Reports are to be completed within 60 days following the last day of each quarter.

How to Submit Dental Provider Quarterly Reports

Quarterly Report Due Dates:

Q1 | January- March 2026 Report due by May 31, 2026

Q2 | April – June 2026 Report due by August 31, 2026

Q3 | July – September 2026 Report due by November 30, 2026

Q4 | October – December 2026 Report due by February 28, 2027

Submit your quarterly report to TCWD:

  1. Online Submission: Please click here to access the online quarterly report. An automatic email will be sent to your practice site contact for confirmation of employment.  The Practice Site Contact should be a practice administrator or office manager.
